South Central Regional Medical Center offers a new service, care management, to patients who have a chronic disease like COPD, diabetes, or heart failure.  Skilled nurses and managers will help patients become educated on their diagnosis, medication, and recovery.  Care Manager, Kerri Drennan, states that the care starts in the hospital and continues when you’re discharged home.  Kerri’s desire to help these paitients started when her son was diagnosed with a terminal disease, and a care manager helped throughout his stay.  The nurses are very well educated, they’re kind, they love their patients, and they love what they do.
