To help ease the fear of living with a chronic disease and help patients navigate through the process, South Central Regional Medical Center offers a quality care management program that offers one on one care.  Care manager Alecia Bigler states that each patient has a unique plan and educating the patients is their main goal as a care manager.  Understanding a disease and how best to care for it allows for a better and healthier quality of life.  Care managers want to educate their patients on their diagnosis, their medications, how and when to take their medications, follow up appointments, and any resources available outside of the community like home health, transitional care services, and medication cost assistance programs.
