South Central Regional Medical Center is committed to working collaboratively to address the healthcare needs of our community. We are asking YOU to help us better understand our community health needs by completing this survey.

WHAT DOES THE SURVEY ASK? 

The survey focuses on how you currently rate our community’s health including access to care, what you see as current obstacles and priorities, what makes a community healthy and more.

There are a handful of demographic questions, but your information will be confidential. If you would like a copy of the survey results, the last question does request more in-depth information, which will also be kept confidential.

HOW WILL THE RESULTS BE USED?

Survey results will be used to develop a comprehensive Community Health Needs Assessment, which describes a community’s health, needs, and the factors that contribute to health. The results can then be used to plan, manage, and evaluate community health initiatives.
